To qualify, the patient must...
- Be under the care of a doctor who agrees to direct your home care services with us
- Require skilled medical services
- Be home bound (if required by your payor source)
For your convenience we accept Blue Cross & Blue Shield Insurance as well as Private Pay, in addition to Medicare and Medicaid.
Ultimate Care does not discriminate in:
